Social Prescribing Link Worker x4 – Calderdale
£24,214 – £27,536 (+ 10% Employers Pension Contribution)
30 -37.5 hours per week. Flexible working and part time working (30 hrs or over) considered
The social prescribing link workers will be employed by Pennine GP Alliance Ltd with direction from Upper Calder Valley, Calder and Ryburn, Central and Lower Valley PCNs and based within the PCN practices. They will play an important role in empowering people to take control of their health and wellbeing, enhancing self-management, active care navigation by receptionists and a greater understanding within practice and by patients of alternative options and support outside of practice available to them.
The roles will involve managing a case load that has both been proactively identified by the social prescribing link worker following interrogation of practice data and/or referred to the social prescribing link workers from the PCNs they work within. The role builds upon and adds value to the existing social prescribing and navigational services available within Calderdale, with whom the social prescribing link works will work collaboratively.
